New patients, as well as some returning with a new injury, need to undergo an hourlong evaluation for their first visit at Pro-Active Physical Therapy. Please arrive 15 minutes early to fill out paperwork. Remember to wear comfortable clothes you can stretch and exercise in. Pro-Active has changing rooms.

We ask patients to send us insurance information prior to the appointment.

Remember to bring:

• Insurance cards
• Photo identification (driver’s license, state ID card, etc.)
• Pro-Active Physical Therapy New Patient Registration Form

Referral form from your physician, if required by insurance company

• Date of injury
• Employer at time of injury
• Claim number
• Adjuster’s name and contact information

Patients filing a legal claim (for example, after a car accident) also need:

• Lien for treatment signed by attorney, if applicable
